    Empowering Autonomy in Person-Centered Care Individuals are not passive recipients of care in person-centered care but active partners in their care journey. Empowering autonomy means involving them in decision-making processes regarding their health, treatment plans, daily activities and lifestyle choices. By valuing their inputs and preferences, healthcare professionals can create care plans that are more aligned with the individual’s goals and desires, promoting a sense of ownership and empowerment which is crucial for successful health journey.

    Empathy: The Foundation of Person-Centered Care Empathy, which is described as the ability to understand and share the feelings of another, is the cornerstone of person-centered care. It involves genuinely listening to clients, acknowledging their fears and concerns and responding passionately. Empathy matters because, it builds trust, reduces anxiety, enhances communication and improves treatment outcomes. By putting clients at the centre of care and approaching each individual with empathy and understanding does not only contributes to a positive environment and foster a culture of compassion but also contributes to making a difference in their lives and this can result to better health outcomes.     Together for change in person-centered care! The Erasmus+ COMPASS project guided us towards ambitious and transformative objectives, focusing on the evolution of European managers in the field of care. Here are our goals: 1. Development of Leadership Skills for Change: COMPASS is committed to providing European care managers with the skills needed to lead change towards the adoption of person-centred care (PCC) approaches in their work contexts. We are on the right track to transform care into a personalized and empathetic experience for everyone. 2. Supporting Reflection and Reflexivity: COMPASS offers care managers a safe space and a methodology to support reflection, reflexivity and learning from their work experience. we are starting to build a network of dedicated professionals, ready to share their professional development process, thus creating the foundations of a deeper and more personalized care. 3. Building of an European Community of Practice (CoP): COMPASS represents a step towards building a solid community of care managers and workers in this field, for person-centered care. We consider sharing experiences and skills, building a common language and consolidating a new and humanistic professional vision of care.     ➡️ COMPASS – Care Managers Leading in Person Centered Care it's an EU funded training program involving 5 other european parteners, that aims to equip European managers with skills to lead the adoption of #PCC approaches. It was a pleasure to colaborate with APROXIMAR Cooperativa de Solidariedade Social on this webinar where we had the opportunity to share our experience on implementing an Age Friendly institution with a scope on leadership and managing change. 🫂And thank you Sandra, Joana Portugal and Jessica for the exchange of ideias and ground experience.
